If anything on this website or at our office is hard to use, please tell us. We will fix it, or we will help you get what you need another way.
Report a barrier by phone
The fastest way to reach us about an accessibility issue is to call the office.

Monday-Thursday, 7:30 AM to 4:30 PM.
Ask for the office manager and say you are calling about the website or an accommodation.
Please have ready, if you can:
- The page address (URL) where you saw the problem, or what you were trying to do
- The browser or device you were using (for example, Chrome on an iPhone, Safari with VoiceOver, Windows with NVDA)
- Your name and a good call-back number or email
What happens after you call
- We acknowledge your report during the call or by call-back within one business day.
- If we can solve it right away - by reading the page to you, emailing you the content in a different format, or scheduling an in-person or phone appointment - we will.
- If it is a fix to the website, we aim to complete it within ten business days. Complex fixes may take longer; we will keep you updated.
- If you would like us to email you when the fix ships, tell us an email address during the call.

Our target standard
We aim to conform our website to Web Content Accessibility Guidelines (WCAG) 2.2 Level AA. We test with a combination of automated scanning, keyboard-only navigation, and the accessibility tree that screen readers use. If you find a place where we have fallen short, your report is how we learn about it.
Downloadable forms are optional
The new-patient PDF and HIPAA Acknowledgement DOCX on our New Patients page are optional conveniences for people who prefer to read paperwork at home. You can skip them entirely and fill everything out on paper when you arrive at the office - just plan on 10 to 15 minutes extra for your first visit. No one is required to use the downloads.
Accommodations at the office
If you need an accommodation to receive dental care - for example, a sign-language interpreter, assistance reading intake forms, extra appointment time, or a ground-floor appointment - please tell us when you schedule. We will arrange it. We do not charge extra for accessibility accommodations. We comply with the non-discrimination provisions of the Americans with Disabilities Act (ADA) and Section 504 of the Rehabilitation Act.
If we have not resolved your concern
If you believe our practice has not met its obligations under the ADA or Section 504, you may file a complaint with the Office for Civil Rights, U.S. Department of Health and Human Services, 200 Independence Avenue SW, Washington, DC 20201.
