This page contains two related documents. Part 1 is the website Privacy Notice, which describes the limited information this website (hiltonheaddentalteam.com) collects from visitors. Part 2 is our federal HIPAA Notice of Privacy Practices, which describes how we may use and disclose your protected health information at our dental office and the rights you have regarding that information.
Important: this website does not collect protected health information
Hilton Head Dental Team does not use this website to collect, transmit, or store patient medical or dental information. There are no contact forms, appointment-request forms, or online patient portals on this website. All patient intake paperwork, including health history and the HIPAA Acknowledgement, is provided as a PDF or DOCX download that you print, complete on paper, and bring to the office in person. If you prefer, you can complete the forms when you arrive. Please do not send health information to us by email; email is not a secure channel for protected health information.

Who we are

This Privacy Notice covers the website hiltonheaddentalteam.com and the data collected through ordinary use of that website.
Information collected automatically
When you visit this website, a small amount of technical information is collected automatically so that pages can be delivered correctly and so we can understand, in aggregate, how visitors find and use the site. This information includes:

This website does not set advertising, tracking, or cross-site cookies. Your browser may store minor preferences locally (for example, how far down a page you have scrolled) but no account or identity cookies are created.
Children
This website is not directed to children under 13. We do not knowingly collect personal information online from children under 13. If you believe a child has provided information to us online, please contact the office and we will delete it.
Security
Pages on this website are served over HTTPS. Because we do not collect patient information through the website, there is no online database of patient records associated with this site. In-office records are maintained under the safeguards required by HIPAA and described in Part 2.

Part 2 — HIPAA Notice of Privacy Practices
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Hilton Head Dental Team is required by the federal Health Insurance Portability and Accountability Act (HIPAA) to maintain the privacy of your protected health information, provide you with this Notice of our legal duties and privacy practices regarding your protected health information, and follow the terms of the Notice currently in effect.
Uses and disclosures of your protected health information
We may use and disclose your protected health information (PHI) for the following purposes without your specific written authorization:
- Treatment. We may use and disclose your PHI to provide, coordinate, or manage your dental care. For example, your dentist may share your treatment plan and X-rays with a specialist if a referral is needed, or with another provider in this office who is involved in your care.
- Payment. We may use and disclose your PHI to obtain payment for the services we provide. For example, we may send claims and supporting documentation to your dental or medical insurance carrier.
- Health care operations. We may use and disclose your PHI for internal operations such as quality assessment, training of students, credentialing of providers, and general business management.
- Appointment reminders and treatment alternatives. We may contact you by phone, voicemail, text message, or postcard to remind you of appointments, or to tell you about treatment alternatives or health-related services that may be of interest to you.
- Individuals involved in your care. When appropriate, we may share PHI with a family member, friend, or other individual you have identified as being involved in your care or payment for your care.
- As required or permitted by law. We may use or disclose PHI when required by law, for public-health activities, to report suspected abuse or neglect, for health oversight activities, in response to a valid court order or subpoena, to law enforcement in limited circumstances, to coroners and medical examiners, for organ donation, for certain research activities, to avert a serious threat to health or safety, for workers' compensation claims, and for other uses specifically permitted by the HIPAA Privacy Rule (45 CFR Part 164).
Uses and disclosures that require your written authorization
Other uses and disclosures of your PHI not described in this Notice will be made only with your written authorization. In particular, the following require your authorization:
- Most uses and disclosures of psychotherapy notes
- Uses and disclosures of PHI for marketing purposes
- Disclosures that constitute a sale of PHI
You may revoke a written authorization at any time by notifying our office in writing, except to the extent that we have already acted in reliance on it.
Your rights regarding your protected health information
- Right to inspect and copy. You have the right to inspect and receive a copy of your PHI in our records, including an electronic copy when readily producible, subject to limited exceptions. We may charge a reasonable fee for copies as permitted by law.
- Right to request an amendment. If you believe the PHI we have about you is incorrect or incomplete, you have the right to ask us to amend it. We may deny your request in certain circumstances and will provide a written explanation if we do.
- Right to request restrictions. You have the right to ask us to restrict how we use or disclose your PHI for treatment, payment, or health care operations. We are not required to agree to every request, but we will agree to a request to restrict disclosure of PHI to a health plan if the disclosure is for payment or health care operations, is not otherwise required by law, and the PHI pertains solely to an item or service for which you have paid us in full out of pocket.
- Right to request confidential communications. You have the right to ask us to communicate with you about your PHI in a specific way or at a specific location (for example, only by mail or only at a daytime phone number). We will accommodate reasonable requests.
- Right to an accounting of disclosures. You have the right to request a list of certain disclosures of your PHI we have made, other than disclosures for treatment, payment, health care operations, and certain other purposes.
- Right to a paper copy of this Notice. You have the right to a paper copy of this Notice on request at any time, even if you have agreed to receive it electronically.
- Right to be notified of a breach. You have the right to be notified following a breach of unsecured PHI.
To exercise any of these rights, please contact our office in writing at the address below. We will provide the forms needed to make a formal request.
Our duties
Hilton Head Dental Team is required by law to maintain the privacy of your PHI, provide you with this Notice of our legal duties and privacy practices regarding PHI, notify you if a breach of your unsecured PHI occurs, and follow the terms of the Notice currently in effect. We reserve the right to change this Notice and to make the revised Notice effective for all PHI we maintain. A copy of any revised Notice will be posted in the office and on this website.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with our office or with the Secretary of the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
